Chapter Awards and Nominations

Florence Kendall dedicated nearly seventy years of her life to physical therapy. She and her husband Henry played a major role in drafting the original bill that was enacted into law in 1947, legally establishing the practice of physical therapy in Maryland. This award is presented at the Fall Meeting. Nominees should be: * Physical therapists or physical therapist assistants who have demonstrated outstanding service to the profession of physical therapy in the area(s) of patient care, supervision, administration, education, and/or research. * Physical therapists or physical therapist assistants who have participated and been a dominant force in activities that have significantly affected chapter or professional growth and development.

The Stephen M. Levine award was established to recognize a member who exemplifies the contributions made to the profession by Stephen M. Levine, PT, DPT, MSHA, FAPTA. Help us honor other Maryland APTA members who continue Florence , Henry, and Steve's quest to advance the profession.

  2. A narrative form, on a separate page, that documents the nominee’s specific activities and contributions and why you feel this qualifies him/her for this award to be presented at the Annual Meeting. 
  3. A current curriculum vitae for the nominee.
